It’s 3 a.m. again. Here is what may be going on.

After 40, broken sleep, lost words, and a shorter fuse often share one cause.

  • Why the three show up together
  • How to tell if perimenopause is part of it
  • What has been tested in women your age

In a few hours you will have to act like you slept. Before you blame yourself, the coffee, or your job, read this. It takes about six minutes, and it may explain more than you expect.

From the author

A note before we start

I am not going to tell you it is all in your head, and I am not going to sell you anything for the next five minutes. First we work out what is going on. Near the end I name the one thing I would try first, and I show my reasons.

01Your week

Does this sound like your week?

A woman lying awake in bed at night, the clock beside her reading 3:02
3:10 a.m.Wide awake. Your mind is already running.
9:30 a.m.The word is right there, and it will not come.
2:00 p.m.You read the same email three times.
6:15 p.m.You snap at someone over nothing.

You can still run a house and a job. Nobody at work can tell. But you can. And somewhere in the third week of it, you start to wonder if something is wrong with you.

What you may be telling yourself

“I am just stressed.” “I am getting older.” “I used to handle things better than this.”

What is more likely

Your hormones started shifting years before your periods stop. Sleep, memory, and mood are the first three things they touch.

You are not losing it. You are losing sleep, and the rest follows.

02The chain

Why the three show up together

In perimenopause your two main hormones stop moving in a steady rhythm. That one change sets off a chain that runs through your whole day.

Hormones swingProgesterone drops. Estrogen spikes and dips from week to week.
Sleep breaksYou sleep lighter, run hotter, and wake around 3 a.m. Falling back asleep gets harder.
Words go missingEstrogen helps the parts of the brain that find words and hold a thought. A short night makes that worse.
The fuse gets shorterEstrogen also helps steady your mood. Add a bad night, and small things feel huge.

↻ and the next night, it starts again

The night sits in the middle of it.

One bad night makes the next day’s words and fuse worse. So when the night gets better, the day usually follows.

This is not a small group of women.

Sleep · SWAN study3,000midlife women followed. Trouble sleeping rose through perimenopause. Waking in the night was the top complaint.
Words · SWAN study2,362women tested for four years. Learning slowed in late perimenopause, then picked back up after.
Mood · Penn study4×more likely to report low mood during the transition than before it, in 231 women over eight years.

03The question you came with

Could perimenopause be part of it?

Perimenopause is the stretch of years before your periods stop for good. Most women are in it longer than they think.

Here is how I think it through in clinic. Three quick questions.

1Are you somewhere between 38 and 55?
2Have your periods changed at all? Closer, further apart, heavier, lighter, or skipped?
3Are the sleep, word, and mood problems new in the last few years, and do they come and go?

Two yeses out of three, and perimenopause is the most likely reason. Not stress. Not your character.

What women often hear“Your bloodwork is normal, so it is probably just stress.”
What a menopause doctor knowsHormone levels swing so much in perimenopause that one blood test can look normal on a bad month. We go by your age, your periods, and your symptoms.
Still see your own doctor if

You snore loudly, feel low most days, or no amount of sleep leaves you rested. Thyroid, low iron, sleep apnea, and depression can look like this.

What you can do today

Write down your last three period dates and the nights you woke. That list tells a doctor more than one blood test.

04Why the first fix fails

One fix rarely covers all three

Most women start with a sleep aid, because the night is the loudest problem. But perimenopause hits four lanes at once.

Nightthe 3 a.m. wake
Moodshort fuse, lost words
Heatflashes, sweats
Gutbloating, new opinions
Most fixes talk to one of these

A sleep aid can knock you out at 10 p.m. and still leave you awake at 3 a.m., because the heat woke you. That is why the first thing you tried helped a little, then let you down. It only covered one lane.

05Your real options

What has actually been tested

Here is the honest list, in the order I go through it with patients. None of these is a cure. Each one is a tool.

Option 1Hormone therapy

The strongest tool there is, if it is right for you.

A prescription drug with real risks: clots, stroke, breast cancer with estrogen plus progestin. Many women cannot take it or do not want to.

Option 2Sleep habits and CBT-I

A cool dark room, the same wake time, no screens in bed. Worth doing.

Does not stop the hormone swing that is waking you.

Option 3Melatonin

Helps some people fall asleep at bedtime.

Less useful for the 3 a.m. wake. Some women wake up groggy.

Option 4Plants with human trials

A handful of plants were tested in women this age, at set doses, against placebo.

Only if the bottle carries the tested plants at printed amounts. These are the ones I look for.

The plants, and what each trial found

These are studies of single ingredients at set doses. They are not a trial of any finished product, and I say so every time.

06The catch

Buying these one at a time has a cost

Two plants for the night. Two for the mood. Two for the heat. Buy them the usual way and this is your counter.

Six unlabeled supplement bottles cluttering a bathroom counter
Six labels to check. Six things to remember at 10 p.m. when you are already tired.
6bottles, six labels, you manage it every night
→
1gummy a day, the same ingredient groups, one label

That row is not self care. It is clutter standing in for a plan.

So I look for one formula that covers the night, the mood, and the heat at once, prints its amounts, and leaves out melatonin.

07The one I would try first

Still You Peri Night Gummies, checked

The only formula I found that covers every lane above in one gummy, with the amounts printed. One a day. 0 g sugar.

Heat · black cohosh, soy isoflavones, red clover, each printed by amount
Night and mood · ashwagandha, saffron, L-theanine, rhodiola. No melatonin.
Botanical support · sphaeranthus, mangosteen, green tea, moringa, gynostemma
200 mgBlack cohosh root
200 mgSoy isoflavones
100 mgRed clover
20BCFU probiotics for the gut lane: L. rhamnosus GG, L. reuteri, L. acidophilus, B. bifidum.

Lane by lane

  • Night. Ashwagandha and L-theanine, the two plants from the sleep trials above. No melatonin, so no groggy morning.
  • Mood and words. Saffron and rhodiola, the two from the mood trials above.
  • Heat. Black cohosh 200 mg, soy isoflavones 200 mg, red clover 100 mg, each on its own line.
  • Gut. Four named probiotic strains, so you do not need a second bottle for bloating.

Where it loses marks

  • The rest blend prints one 500 mg total for four plants, so you cannot see the exact amount of ashwagandha or saffron.
  • There is no trial of the finished gummy. That is true of every supplement I have ever checked, and I will not pretend otherwise.
  • It contains soy. If you have a soy allergy, this one is not for you.

I count those against it. It is still the only bottle I found where the night, the mood, and the heat are all covered by plants tested in women this age.

08The cost of waiting

What another month of 3 a.m. costs

Waiting to decide feels free. It is not. Here is what the same 30 nights look like, both ways.

If you wait another month

30 more nights of waking up at 3 a.m.

30 more mornings you start already tired, already hunting for words, already short with the people you love. Nothing changes, because nothing was started.

If you start tonight

The same 30 nights, spent building.

Plants like these work slowly, night after night. By week 8 the ashwagandha study had its answer. By week 12 the saffron study had its answer. So will you.

Week 0

First gummy the night it arrives.

Week 8

Length of the ashwagandha trial.

Week 12

Length of the saffron trial. Decide here, not on night two.

You came here to find out why. Now you know. Staying the woman who almost started is usually the expensive choice.

I was already juggling three bottles at once. No fireworks. No overnight miracle. What I got instead was one simple thing I actually keep taking. The 3 a.m. wake-ups eased up. The days stopped feeling like I was dragging a stranger around in my own body.

★★★★★Ruth, 51July 21, 2026

My husband kept saying I had changed. I was mean. I was always mad. I could not tell if I was crazy or justified. Then 3am would come and I would be up for good, sometimes changing my shirt, sometimes just staring at the ceiling.

The wakeups ended. The hot flashes eased. I can get through a conversation without the mood swinging out from under me. I missed me. I am not missing her anymore.

★★★★★Heidi J.August 22, 2026 · Verified purchase

I read labels before I buy anything, especially supplements. I checked the probiotic blend and liked that I could actually see what I was taking. I gave it a full month before judging it, and I’m getting longer stretches of sleep.

The brand page listed 1,191 reviews at a 4.9 average when I updated this piece. A star average is not a lab result.

10Is it for you

Fit and the usual cautions

Worth a closer look if you

  • Are between 38 and 55 and your periods have changed
  • Wake around 3 a.m. and cannot get back down
  • Want non-hormonal support
  • Want one daily routine instead of a row of bottles

Ask your own clinician first if you

  • Take prescription medication, including HRT
  • Have a liver that is monitored
  • Are pregnant, nursing, or trying to conceive
  • Have a soy allergy, since this formula contains soy

11Last word

If it were my sister

I would tell her the 3 a.m. wake, the lost words, and the short fuse are one problem, not three. Then I would send her this one bottle and tell her to take it for the time the studies used, about 90 days, not a long weekend. The guarantee is there if it is not right.
